Singapore's healthcare system is staring down a demographic reality: an ageing population with increasingly complex medication regimens, served by pharmacists who spend over 1,000 combined hours each week on manual reconciliation, triage, and verification work. The National University Health System (NUHS) has now deployed an AI platform designed to shift that equation — automating the repetitive tasks that consume pharmacists' time so they can focus on the clinical judgment that machines cannot replicate.

The NUHS Cluster AI in Pharmacy (NCAIP) platform brings together four AI tools that handle medication reconciliation, triage, and verification across the cluster's pharmacy services. These processes — matching a patient's medication list across care transitions, assessing prescription complexity, and verifying inpatient orders — currently consume roughly 350 hours per week on admission and discharge reconciliations alone, plus 190 hours on routine triage and 310 hours on inpatient order verification. NCAIP targets these workflows directly, using AI to surface discrepancies, flag potential safety issues, and prioritise cases that need pharmacist attention.

The platform is built on NUHS's existing ENDEAVOUR AI system, which already enables real-time data integration across the cluster's hospitals and polyclinics serving approximately 1.2 million residents in Singapore's western region. What distinguishes NCAIP from point-solution AI tools is its design as a scalable platform — new pharmacy AI modules can be piloted and introduced over time, expanding the system's capabilities without rebuilding the underlying infrastructure. This platform approach mirrors Singapore's broader healthcare AI strategy, where integrated data systems like ENDEAVOUR create the foundation on which specialised AI applications can be built.

By reducing time spent on manual and administrative processes, the platform enables pharmacists to expand into higher-value clinical areas including pharmacogenomics — using genetic information to predict drug responses — and leading transdisciplinary care for chronic conditions such as heart failure, diabetes, and renal disease. The scale of redeployment is significant: even a 30% reduction in manual verification hours across NUHS would free up the equivalent of 15-20 full-time pharmacists for clinical work, at a time when Singapore's healthcare workforce is under increasing strain from an ageing population and rising chronic disease burden.

Why it matters for Singapore: NCAIP represents the kind of AI deployment Singapore needs to scale if it wants to maintain healthcare quality while managing costs and workforce constraints. The platform approach — building on existing data infrastructure rather than creating a standalone tool — is also the template that other clusters like SingHealth and NHG could follow. As Health Minister Ong Ye Kung has signalled, AI in healthcare is not about replacing clinicians but about giving them back time for the work that requires human expertise. NCAIP is a concrete demonstration of that principle at cluster-wide scale.
